National Rural Health Association and Stratis Health are excited to invite you to participate in a discussion forum and idea exchange regarding opportunities to leverage Rural Health Transformation Program implementation to support improved health and care for older adults in your state. This event is designed for Rural Health Transformation Program leaders at a state level to identify and discuss incorporation of age-friendly strategies as part of your CMS-approved RHTP plans. Whether you are with a state office of rural health, a state Medicaid agency, a state rural health association, or will have a role or influence in how your state’s RHTP will be implemented, if you are interested in supporting age-friendly approaches, this is the forum for you to listen, learn, and share!
What Participants Will Experience
- Hear from and share with other state leaders ways in which age-friendly strategies are or could be included as part of RHTP plans
- Discuss opportunities and mechanisms for ensuring that RHTP activities incorporate strategies that address the needs of older adults in rural communities
- Identify tools and resources that support incorporation of age-friendly best practices into RHTP activities
